RELEASE FORMS

Please make sure that you get a signed copy of all necessary release forms. Please also make sure that the contributor and location details are written in CAPITALS with how they would wish to be captioned, that the date is clear and that all address and telephone numbers are written on to the form – this is very important for captioning and for ease of contacting contributors later on in the production.

Health and Safety - NOTES FOR CREW

Please contact a member of the Production Team at any time with any Health and Safety issues.  All crew to be aware of their equipment at all times.  Production Health and Safety is a very important issue and it is everyone’s responsibility. All Accidents to be reported to the Production Manager and logged on Accident report form.

13:30	Walk back to my house.
14:30	The filming day ends. Connor walks home.
